If you have lost or found an animal and would like to report it to Animal Control please fill out the below form.

Owner Surrender Your Animal

If you are no longer able to care for your animal and are a resident of Addison, please contact Animal Control to discuss the issue and set up an appointment to surrender your animal. There is a $70 surrender fee that must be paid at the time of surrender. Please bring the filled out form below with you, along with any veterinary records you have for the animal.
